How to Translate and Transfer Vaccination Records for the I-693

If your vaccination records are in a language other than English, they must be translated by a certified translator before your Civil Surgeon can use them. Records from your home country don’t need to be on a US-style yellow card — just legible, dated, and translated.

What the Civil Surgeon needs to see

  • Vaccine name (generic, not brand)
  • Date of administration
  • Healthcare provider or clinic name
  • For multi-dose series, the dose number

Where to get translations

USCIS-acceptable certified translations are widely available online (0–$60 per page). The translator must sign a Certificate of Translation Accuracy.

What if I have no records?

You’ll need to get the required vaccinations at your Civil Surgeon’s visit — or earlier with your primary care doctor and bring proof. Without records or current vaccinations, the I-693 cannot be completed.
