Every required vaccine, every age requirement, every waiver — current as of 2026.
The required vaccines
USCIS requires the following vaccinations for every adult I-693 applicant. If you can document complete prior immunization, the Civil Surgeon records those dates on the form. If not, they administer the missing doses at your exam visit, billed at cost.

If you can't get a required vaccine
Three blanket waivers can be applied directly on the I-693 by your Civil Surgeon:
- Not age-appropriate — vaccine isn't recommended for your age
- Medically contraindicated — documented allergy, immunocompromised, pregnancy (for live vaccines)
- Not routinely available — rarely invoked in the U.S.
For religious or moral objections, file Form I-601 separately.
