Every green-card applicant adjusting status inside the US needs a USCIS Form I-693 from a designated Civil Surgeon. Here's what you need to know.
USCIS requires the I-693 for almost every adjustment of status
If you are filing Form I-485 (Application to Register Permanent Residence or Adjust Status) inside the United States, you need a Form I-693 medical examination performed by a USCIS-designated Civil Surgeon. Your family doctor cannot do this — only Civil Surgeons on the USCIS list are authorized.
If you are consular processing (applying for an immigrant visa at a US Embassy or Consulate abroad), you complete the equivalent Form DS-2054 with a Panel Physician approved by that embassy. Documents you need to bring
- Government-issued photo ID (passport is best; US state ID also accepted)
- USCIS receipt notice for your I-485, if you have one yet
- Complete vaccination history — bring everything (childhood records, school records, military records, international yellow card)
- List of current medications with dosages
- For children: pediatric vaccination record + birth certificate
