Our mission

Most immigrants going through Adjustment of Status discover the medical exam too late. They call three Civil Surgeons, get three different prices, hear three different things about what’s included, and end up paying $450 to a clinic that mails the unsealed envelope back to them on the wrong form. We exist to short-circuit that experience. Our mission is simple: match every applicant with a Civil Surgeon (and, where needed, an immigration attorney) who meets a published service standard, at a transparent price, in less than one business hour.

How we vet providers

Every clinic in our network is independently verified against five criteria before we accept them. We re-verify every 12 months.

Geographic coverage

We currently maintain provider relationships across all 50 states and the District of Columbia, with active matches in more than 300 metropolitan areas. Concentration is highest in the top 20 immigrant-receiving metros (New York City, Los Angeles, Houston, Chicago, San Francisco, Miami, Dallas, Boston, San Diego, San Jose), where we typically have 4–8 vetted Civil Surgeons within a 25-mile radius. Smaller metros have at least one designated provider; some rural states (Wyoming, North Dakota, Vermont) require travel to the nearest large city, and we are transparent about that during matching.

How matching works

The matching process takes about 90 seconds of your time and 60 minutes of ours:

1. You submit a short form. Name, email, phone, city, and a one-line case description (AOS spouse, employment-based, asylum, etc.).
2. We identify candidates. Our team filters the network by your location, your case type, your preferred language, and the providers currently accepting new patients.
3. We call you back. Usually within one business hour during 8am–8pm EST, Monday through Saturday. We confirm scope, explain options, and answer questions.
4. We send introductions. Once you choose a provider, we send a warm introduction by email or phone and the provider's office reaches out to schedule. You take it from there.

What makes us different from booking direct

You can absolutely call a Civil Surgeon directly. Many people do. Here’s what you give up:

• Pricing leverage. Network clinics give us their best published rate. You can sometimes match it by calling around, but it’s 5–10 hours of phone work.
• Independent vetting. A clinic’s own website tells you what they want you to know. Our SLA tracking tells you whether they actually hit five days last quarter.
• Recovery path if something goes wrong. If a network clinic misses the SLA, we can escalate. If you booked direct, your only recourse is a Yelp review.
• Cross-referrals. Most cases benefit from a Civil Surgeon AND an immigration attorney. We can introduce both in the same conversation.

The service is free to applicants. We receive a per-match referral fee from network providers, which is disclosed up front and does not affect the price you pay at the clinic.

Founding story

The network started in 2022 after our founder spent three months trying to schedule his own I-693 exam in a major U.S. metro. He called eleven Civil Surgeons. Four quoted prices over $500. Two refused to disclose pricing without an in-person consult ($75 just to get a quote). One agreed to $295, but only when paying cash, in person, on the day. The sealed envelope took 18 days to arrive. The form had two checkboxes wrong, triggering an RFE that delayed the case six months. By the time he had a green card in hand, he had built the spreadsheet that became the first version of the network: a list of Civil Surgeons whose pricing, turnaround, and accuracy met a published standard. Today that spreadsheet has 300+ verified entries and a team that picks up the phone within 60 seconds during business hours.
