Vaccinations for the I-693
USCIS requires specific vaccinations per the CDC Civil Surgeon schedule. We stock all of them and administer same-day at your exam visit — charged at cost, no admin fee.
Required vaccines (every adult applicant)
If you can document a complete series of any of these from your past medical history, we don't re-administer — we record the dates from your records onto the I-693. If you're missing doses or have no records, we administer the dose at your exam.

Age-specific add-ons
- Hib (Haemophilus influenzae b) — under 5 years old
- Rotavirus — 6 weeks to 32 weeks of age
- Pneumococcal (PCV) — under 5 or 65+
- Meningococcal (MenACWY) — 11-18 years old
- HPV — 11-26 years old (catch-up through age 45)
If you can't or don't want a required vaccine
Three blanket waivers can be applied directly on the I-693 by your Civil Surgeon:
- Not age-appropriate — the vaccine isn't recommended for your age
- Medically contraindicated — documented allergy, immunocompromised status, etc.
- Not routinely available — rarely invoked
For religious or moral objections, you'll need to file Form I-601 separately with USCIS. We can guide you through that process — it requires additional paperwork demonstrating the sincerity and consistency of the belief.
