USCIS uses the CDC’s Vaccination Schedule for Civil Surgeons to determine which vaccinations are required at your I-693 exam. The 2026 list:
Always required (all applicants)
- MMR (measles, mumps, rubella) — 2 doses or proof of immunity
- Polio (IPV) — full childhood series or 1-dose booster as adult
- Tetanus, diphtheria (Td) or Tdap — most recent dose within 10 years
- Varicella (chickenpox) — 2 doses or proof of immunity
- Hepatitis B — full 3-dose series or proof of immunity
- Seasonal influenza — if exam is between Oct 1 and Mar 31
- COVID-19 — currently required per October 2021 USCIS update
Required at specific ages
- Hib (Haemophilus influenzae type b) — under 5 years old
- Rotavirus — 6 weeks to 32 weeks
- Pneumococcal (PCV) — under 5 or 65+
- Meningococcal (MenACWY) — 11–18 years old
- HPV — 11–26 years old (catch-up to 45)
What if I can’t get a vaccine?
Three blanket waivers are available: too young, medically contraindicated, or not age-appropriate. A fourth category — religious or moral conviction — requires an additional USCIS Form I-601 application. See our guide to vaccination waivers.
