I-693 Appointment Checklist: What to Bring

Bringing the right documents on day-one cuts a typical I-693 visit from 90 to 45 minutes. Here’s the checklist we send to every patient.

Required

  • Government-issued photo ID (passport preferred; state ID acceptable for US-issued)
  • USCIS receipt notice for your I-485 application (if available)
  • Vaccination history — childhood records, employer or school records, or international yellow card; certified English translation if in another language
  • Cash or card for payment in full at the visit
  • List of current medications including dosage
  • Eyeglasses or contact lenses (you’ll be asked about vision)
  • List of past surgeries and major illnesses
  • For applicants over 65: copy of recent EKG if available

For children

  • Parent ID
  • Child’s birth certificate
  • Child’s pediatric vaccination record
  • If applicable: child’s USCIS receipt notice

Once you have everything together, you can review or book the I-693 exam directly.
