Form I-693, Report of Medical Examination and Vaccination Record, is the form your Civil Surgeon completes after your immigration medical exam. It’s required for every adjustment-of-status case and for some other immigration benefits.

What the exam includes

  • Physical examination (height, weight, vital signs, head-to-toe physical)
  • Tuberculosis screening — IGRA blood test if you’re 2 years or older
  • Syphilis screening — serologic blood test for everyone 15 and older
  • Gonorrhea screening — urine NAAT for adults 18–24 and others by risk
  • Mental health and substance use evaluation (verbal screening)
  • Review of vaccination history and administration of missing required vaccines

What the exam does NOT include

The I-693 is not a general physical. Your Civil Surgeon will not screen for or treat conditions outside the USCIS-mandated list. If you have ongoing health concerns, schedule a separate visit with your primary care physician.

2026 validity rules

USCIS Policy Manual update PA-2023-26 made I-693s indefinitely valid (no longer the old 2-year window) provided the Civil Surgeon signs on or after November 1, 2023 and the exam was performed within 60 days of, or any time during, the I-485 pending period.

If you’re ready to schedule, our I-693 exam service page covers what a visit includes and how to book.
