USCIS maintains the official Civil Surgeon directory at my.uscis.gov/findadoctor. Search by ZIP code; results show clinic name, address, phone, and a generic note about pricing.

How to choose

  1. Confirm current designation — Civil Surgeon designations expire and require renewal. The USCIS directory is the source of truth.
  2. Ask about all-in pricing — base fee, sealed envelope, document handling. Get a quote in writing.
  3. Confirm same-day vaccination availability — most clinics stock common vaccines; some require a follow-up visit.
  4. Check turnaround time — typical is 5 business days for the sealed I-693; some practices offer rush at extra cost.

Red flags

  • Won’t quote a price over the phone
  • Asks you to bring a non-USCIS form or wants you to fill out only part of the I-693 in advance
  • Doesn’t seal the envelope in front of you
  • Open dispute on USCIS Civil Surgeon designation registry

When you’re ready to compare options, you can browse our civil surgeon directory to get started.
