Civil Surgeons set their own fees, so prices vary widely. Based on our 2026 survey of 312 USCIS-designated practices across the US, the median I-693 exam fee is $295, with a 25–75 percentile range of $240–$385.

What’s included in the base fee

A reputable practice’s base fee covers the physical exam, syphilis serology, gonorrhea NAAT, mental health and substance use screening, TB IGRA blood test, and the sealed I-693 envelope.

Add-on costs to expect

  • Missing vaccinations — $45–$165 per dose depending on the vaccine
  • Chest X-ray (only if your TB IGRA is positive) — $95–$180
  • Records translation if your vaccination history is in a non-English language — $50–$120

Hidden fees to watch for

Some practices charge “file processing” or “envelope sealing” fees of $50–$100 on top of the exam fee. Ask for an all-in quote before booking.
