If you’re getting a green card through marriage to a US citizen and currently in the US, you’ll complete Form I-693 with a USCIS Civil Surgeon. If you’re in your home country waiting for a US Embassy interview, you’ll complete Form DS-2054 with a Panel Physician approved by the embassy. Same exam, different forms, different physicians.
Form I-693 (adjustment of status)
- Filed with Form I-485 inside the US
- Completed by USCIS-designated Civil Surgeon
- Sealed envelope submitted to USCIS
- Indefinitely valid (since Nov 2023 update) if conditions met
Form DS-2054 (consular processing)
- Required for the visa interview at a US Embassy or Consulate
- Completed by an embassy-approved Panel Physician
- Results transmitted electronically to the embassy
- Valid 6 months from exam date
